# Điều 14. Hồ sơ quản lý chất thải y tế

Cơ sở y tế phải lưu giữ tại cơ sở các hồ sơ sau đây:

1\. Giấy phép môi trường và các văn bản, hồ sơ, tài liệu có liên quan về môi trường theo quy định (đối với cơ sở y tế thuộc đối tượng phải xin cấp giấy phép môi trường).

2\. Sổ giao nhận chất thải y tế; chứng từ chất thải nguy hại (nếu có).

3\. Sổ nhật ký vận hành công trình, thiết bị, hệ thống xử lý chất thải y tế (đối với cơ sở y tế tự xử lý chất thải).

4\. Các biên bản thanh tra, kiểm tra liên quan (nếu có).

5\. Báo cáo kết quả quản lý chất thải y tế định kỳ hằng năm; báo cáo kết quả quan trắc chất thải định kỳ (nếu có).

6\. Các tài liệu liên quan khác.
