# Điều 13. Chế độ báo cáo

1\. Tần suất báo cáo: Báo cáo kết quả quản lý chất thải y tế được lập 01 năm một lần, tính từ ngày 15 tháng 12 năm trước kỳ báo cáo đến ngày 14 tháng 12 của năm thực hiện báo cáo.

2\. Hình thức báo cáo: Báo cáo kết quả quản lý chất thải y tế được gửi bằng văn bản giấy hoặc bản điện tử hoặc qua phần mềm báo cáo.

3\. Nội dung và trình tự báo cáo:

a) Cơ sở y tế báo cáo kết quả quản lý chất thải y tế của cơ sở theo mẫu quy định tại Mục A Phụ lục số 07 ban hành kèm theo Thông tư này, gửi về Sở Y tế tỉnh, thành phố trực thuộc Trung ương (sau đây gọi tắt là Sở Y tế) trên địa bàn hoặc Bộ Y tế (Cục Quản lý môi trường y tế) đối với các cơ sở y tế trực thuộc Bộ Y tế trước ngày 16 tháng 12 của kỳ báo cáo;

b) Sở Y tế tổng hợp, báo cáo kết quả quản lý chất thải y tế trên địa bàn theo mẫu quy định tại Mục B Phụ lục số 07 ban hành kèm theo Thông tư này, gửi về Bộ Y tế (Cục Quản lý môi trường y tế) trước ngày 20 tháng 12 của kỳ báo cáo.
