# Điều 33. Hồ sơ đăng ký kế hoạch bảo vệ môi trường

1. Hồ sơ đăng ký kế hoạch bảo vệ môi trường thuộc thẩm quyền xác nhận đăng ký của Sở Tài nguyên và Môi trường gồm:

   a) Ba (03) bản kế hoạch bảo vệ môi trường với trang bìa và yêu cầu về nội dung thực hiện theo mẫu tương ứng quy định tại các Phụ lục 5.4 và 5.5 Thông tư này;

   b) Một (01) báo cáo đầu tư hoặc phương án sản xuất, kinh doanh, dịch vụ.
2. Hồ sơ đăng ký kế hoạch bảo vệ môi trường thuộc thẩm quyền xác nhận đăng ký của Ủy ban nhân dân cấp huyện gồm:

   a) Ba (03) bản kế hoạch bảo vệ môi trường với yêu cầu về cấu trúc và nội dung theo mẫu quy định tại Phụ lục 5.6 Thông tư này;

   b) Một (01) báo cáo đầu tư hoặc phương án sản xuất, kinh doanh, dịch vụ của chủ dự án.
3. Trường hợp đăng ký kế hoạch bảo vệ môi trường tại cơ quan được ủy quyền, hồ sơ được thực hiện theo quy định tương ứng với hồ sơ đăng ký tại cơ quan có thẩm quyền.
