# Điều 7. Hồ sơ, biểu mẫu chấp thuận nội dung về phương án chuyển nước theo qđ tại NĐ136/2025/NĐ-CP

Tổ chức, cá nhân nộp 01 bộ hồ sơ đến Cơ quan được Chủ tịch Ủy ban nhân dân cấp tỉnh giao tiếp nhận hồ sơ giải quyết thủ tục hành chính bằng hình thức trực tiếp hoặc môi trường mạng hoặc qua dịch vụ bưu chính. Hồ sơ gồm:

a) Văn bản đề nghị chấp thuận nội dung về phương án chuyển nước theo Mẫu số 03 tại Phụ lục II kèm theo Thông tư này;

b) Báo cáo thuyết minh đề xuất phương án chuyển nước theo Mẫu số 04 tại Phụ lục II kèm theo Thông tư này;

c) Sơ đồ vị trí dự án chuyển nước;

d) Tài liệu pháp lý và kỹ thuật khác có liên quan đến dự án.
