# Điều 22. Hồ sơ, thủ tục đăng ký môi trường

1\. Hồ sơ đăng ký môi trường gồm:

a) Văn bản đăng ký môi trường của chủ dự án đầu tư, cơ sở theo quy định tại Mẫu số 47 Phụ lục II ban hành kèm theo Thông tư này;

b) Bản sao Quyết định phê duyệt kết quả thẩm định báo cáo đánh giá tác động môi trường của dự án đầu tư, cơ sở (nếu có).

2\. Chủ dự án đầu tư, cơ sở gửi hồ sơ đăng ký môi trường đến Ủy ban nhân dân cấp xã nơi triển khai dự án đầu tư, cơ sở thông qua hình thức gửi trực tiếp, qua đường bưu điện hoặc bản điện tử thông qua hệ thống dịch vụ công trực tuyến theo quy định tại khoản 3 Điều 49 Luật Bảo vệ môi trường.
