# Điều 17. Hồ sơ cấp, gia hạn, điều chỉnh nội dung giấy phép thăm dò nước dưới đất

1\. Hồ sơ đề nghị cấp giấy phép thăm dò nước dưới đất bao gồm:

a) Đơn đề nghị cấp giấy phép;

b) Đề án thăm dò nước dưới đất đối với công trình có quy mô từ 200 m³/ngày đêm trở lên; thiết kế giếng thăm dò đối với công trình có quy mô nhỏ hơn 200 m³/ngày đêm.

2\. Hồ sơ đề nghị gia hạn, điều chỉnh nội dung giấy phép thăm dò nước dưới đất bao gồm:

a) Đơn đề nghị gia hạn hoặc điều chỉnh nội dung giấy phép;

b) Báo cáo tình hình thực hiện các quy định trong giấy phép.

3\. Mẫu đơn, nội dung đề án, báo cáo được lập theo Mẫu 01, Mẫu 02, Mẫu 35, Mẫu 36 và Mẫu 37 tại Phụ lục kèm theo Nghị định này.
